Medicare
Part D - Prescription
Coverage for Seniors
Please read this page carefully. It
contains vital information you will need for
completing any ONLINE enrollment.
The
Government has established a fourth part to
the Medicare Program, called Part D, to help
seniors pay for the high cost of
Prescription Medications. So, we now have:
Part A : Hospital Coverage
Part B : Doctor Coverage
Part C : Medicare Advantage Plans, like
HMO’s and PFFS’s
Part D : Prescription Medication Coverage
Part D is set to begin on January 1, 2006
and is available to anyone who currently has
Medicare Part A or Part B. The Initial
Enrollment Period begins on November 15,
2005 and continues until May 15, 2006.
Enrollment in Part D is optional, but it is
HIGHLY
recommended for all who qualify.
The premium for Part D will vary by plan and
by state. But since this program is
Approved by the Government, your monthly
premiums can be deducted directly from your
Social Security check, just like Medicare
Part B premiums. This payment method is
recommended.

Depending on your state of residence, you
will need one of the REPRESENTATIVE CODES
below for the ONLINE enrollment:

When you visit
www.RxPathway.com
to complete your ONLINE enrollment, you will be asked:
“Has
a plan representative assisted you with the
Prescription Pathway online enrollment
form?”
you must answer
YES
and enter
one of the above numbers
(4654XX or
B25036).
This number will guarantee your enrollment
application a quicker and more efficient
turn around time.
Before you are asked the above question, you will be offered two choices for paying
premiums:
Option 1
is automatically deducted from
your social security just like Medicare
Part B. With this method the ENROLLMENT can be done ONLINE.
If you choose Option 2, automatic deduction
from your bank account, you MUST download,
print, fill out, and mail your enrollment
form in which may take substantially longer
to process. ONLINE ENROLLMENT can only be done if you choose
Option 1
(automatic deduction from your social
security). With this process, Medicare will
then mail you a new Medicare ID card showing
Part D coverage.
You also will be asked to choose between different Part D plans.
Below is a partial review.*
The BRONZE plan is the most basic. You must
pay 25% of prescriptions plus a deductible.
The SILVER plan is better. You only pay a
co-pay for each prescription plus a
deductible.
The GOLD plan works like the SILVER plan but has NO DEDUCTIBLE.
The PLATINUM plan is similar to the GOLD
plan with NO DEDUCTIBLE and has enhanced
prescription coverage (Platinum plan
available in certain states only).
We highly recommend the SILVER or GOLD plan for
best coverage value.
One of the many highlights of our plans is
that you are allowed to get a ninety (90)
day supply of medication for the cost of the
sixty (60) day supply. ONE MONTH ABSOLUTELY
FREE.
This means that your medications cost LESS
if you buy a 3 month supply instead of a one
month supply, saving you money and
unnecessary trips to the pharmacy.

SPECIAL NOTE:
If you choose NOT to enroll in Part D
during the Initial Enrollment Period you
will have to wait until the Annual
Enrollment Period which will be November 15th
through December 31st each year
AND you will be charged a penalty of
1% per month for each month you chose not to
participate in Part D. So, if you waited 1
year to apply for Part D, you will pay an
extra 12% a month in premium for the
rest
of your life. The only exceptions to
the penalty are for people who have some
form of creditable prescription coverage now
and want to keep it (you have to call your
carrier to see if it is creditable
coverage). If you drop or lose this
coverage later you will not be charged a
penalty to apply for Part D.
